The db info can be 100% and still fail for two reasons:
- the db user does not have ALL permissions on the db
- on some hosts, if the db user's password is 6 characters or less, it dies.

Going back to 2.8.6 and installing worked fine. There's some error in 2.9.1 that's inherently causing this.
I think I got it, in WPMU the first field is not the server address like in WP, it's 'Server Name'!!!! The last field is the ADDRESS!!!

Here is my 2 cents:
I had the same problem, same error. Try to change the following setting in phpmyadmin:
-In Privilege, instead of "%" as host, replace by "localhost". To do this, click on Action, then change host, and click Go.
