Installation - WPMU Doesn't Create All Tables (2 posts)

This is my first stab at using WPMU, though I'm experienced in standard WP. I went through the install.php file without incident and I know that MU created several tables in my database. When I go to my site (http://www.fusionaz.com/mu), I get this message:
    Database Tables Missing.
Database tables are missing. This means that MySQL is either not running, WPMU was not installed properly, or someone deleted wp_site. You really should look at your database now.
It goes on to tell me to check if these tables are present:
    wp_blogs
    wp_users
    wp_usermeta
    wp_site
    wp_sitemeta
    wp_sitecategories

None of them are in my database, though I do have many other WP tables including Posts, comments, etc. I've deleted the tables and tried to reinstall twice. The result is the same; some wp_ tables are created, but not the ones mentioned above. Am I doing something wrong? Let me know if you have any advice. Thanks!

    Things to check:

    - the db user has ALL permissions on the database
    - the db password is at least 6 characters long
