But also not centered when viewed by me on IE 6.0, not my usual computer. Haven't checked with Firefox.

try maximizing the screen? it looks like that on firefox too :x. Think it's fixed width on the left side?

err.. or increasing your screen resolution. I can't tell :x.. I have widescreen lol

Yes, the full page was viewable at 1600 x 1200, but it was off the screen for 1024 x 768, I believe many people still use that screen size, as far as I know.

Just things to bear in mind, I'll stop with these picky details...

Worked fine for me in FF and the IE tab. I even resized.

That screenshot looks like the form didn't fully load.

The centering issue has now been fixed. I had a typo in the CSS file.

Hmm, the form took me a second to fully open up as well. That was strange. My AUP/TOS has disppeared as well.
