WordPress MU

Hi,
I am wondering how to make a contact form appear on my blog. Since WordPress Mu 1.5.1 is editing out the html, is their something I can do to fix that. Or should I make a theme and upload that to my server with a contact theme.
Well basically, what should I do to create a theme similar to the one on WordPress.Com so like I need a custom website theme.
