Moving my individual blogs to a WPMU setup, using a Windows Server.
The install went fine, and I have the primary blog up and running. However, when I create a new blog, I get a 404 when trying to visit the site. And, the backend isn't created.
Any ideas where to check for problems?

On the windows server are you using apache or trying to use IIS?

I go through this problem alike. After I created one new blog, I can't access its backend. However I can make some settings on the 'edit' page.
Did you follow the instructions listed in the included readme file concerning the wildcard DNS setup? I have a feeling that you didn't. You may want to give the readme file a read though.

Thanks for the quick reply. I config the conf file and the site at last works well on the local server. But the remote sever doesn't support the rewrite module. Can wpmu get pages like this: index.php?blog_index=xxx ?

Not that I'm aware of although I;m sure something like that could be coded. You'll have problems with permalinks as well for Posts and Pages if you don;t have mod_rewrite installed.
