I've just made the initial release of this one:
    http://blog.clearskys.net/plugins/enquiry-contact-form/

It's not fully tested on Wordpress MU yet, but works on Wordpress 2.x
I'm still writing the documentation for it, but more details can be found at the following locations:
    http://blog.clearskys.net/2007/01/16/first-looks-enquiry-contact-plugin/
    and
    http://blog.clearskys.net/2007/02/16/enquiry-and-contact-plugin-01a/
If anyone tries it on MU and has problems (or not), please let me know

That's very cool but anything that requires theme hacks doesn't roll very well with WPMU - we need backend options for inserting / configuring etc.

Yeh, that's ace - I use it for a couple of my own sites. Does it play nicely with WPMU?

Yes, it does. Although I made a few modifications to it. Mainly so that users couldn't change certain elements of it. Like path to the verification file. I also set it to use a different css path, that call the file from the theme vice the plugin directory.
