If you or one of your clients are importing a wordpress import file, turn off the sitemap generator. It generates a new sitemap for each and every post during the importer. :)
edit: Gets you blackballed by Google as well. :(

Umm... hoow / where is this option for the sitemap turn on or off? I've looked around and have not found one..

It's actually a plugin, not part of core. if you have it, it should be found at Dashboard -> Options -> Sitemap.
